Be nice to me

A survey fragment, with responses of "Poor", "Fair", "Good", "Very good" and "Excellent".

The scale is not balanced. There is one choice for an unambiguously negative response (“poor”), one amgibuous choice (“fair” – which can be interpeted as negative, neutral, or positive), and three choices for positive responses (“good”, “very good”, “excellent”).

A better choice would be something like “very good”, “good”, “neutral”, “poor”, “very poor”. There’s a clear neutral point (some people don’t like to provide a neutral, but that’s another story), and each positve has a corresponding negative.
